Pathway Mapping

We are using pathway mapping (in Microsoft Visio) to provide detailed and high-level views of the care system and its interactions. This gives a useful means of comparison which encourages stakeholder participation in questioning current practice and identifying options for improvement. The figure below illustrates the mother’s inpatient care at a typical NHS health board in Scotland. It shows the care which a mother will receive as she moves through the postnatal pathway, from admission to the postnatal ward following delivery, through to being discharged home with her baby.

NHS staff use the SBAR (Situation - Background - Assessment - Recommendation) communication format.  This is a structured method for framing conversations requiring a clinician's immediate attention and action, which helps to improve patient safety.
